Case Manager

Primary Purpose

To provide case management services to program participants.

Essential Duties

  • Conduct intake for new program participants;
  • Conduct assessment for program participants’ needs;
  • Assist program participants with completing and signing appropriate consent forms;
  • Assist program participants in developing individual plans with goals and objectives;
  • Create case files for program participants;
  • Manage cases;
  • Maintain case files;
  • Document records in a timely and appropriately manner;
  • File documents in a timely manner;
  • Evaluate the program participants’ goals and objectives and update them on a regular basis;
  • Conduct regular contact follow up with program participants, face to face, telephone call, or email;
  • Write and submit quarterly progress report on caseload;
  • Manage the warm line and respond to inquiries in a timely manner;
  • Provide acculturation trainings for service providers/community organizations;
  • Provide language/cultural translation for individual cases.

Other Duties

Other duties as assigned including, but not limited to translating forms and interpreting for program participants as appropriate.

Work Hours

  • Monday through Friday, 8:00 a.m. to 4:30 p.m. or as otherwise determined by the supervisor;
  • Position requires regular and predictable attendance;
  • Some events on evenings or weekends at various times.

Working Conditions

  • Office setting; 
  • Travel in personal vehicle to and from meeting sites;
  • Travel to the neighbor islands may be required.

Equipment Use

Personal vehicle, computer and/or lap top, printer, scanner, copier, fax machine, office and/or work cell phone/Bluetooth, paper cutter, nametag maker, voice recorder, speaker system, video and camera equipment, microwave, ice machine, and stove.

Mental:
  • Consistent adherence to the practice of PIDF’s five core values that are used as a guiding force in the success of all of its programs;
  • Be keenly aware of and attentive to program participants;
  • Must analyze and problem solve issues;
  • Must be able to multi-task and prioritize;
  • Must possess strong interpersonal skills;
  • Must be able to cope with stress that may be induced by the situations of our clients.
Physical:
  • Requires lifting and carrying up to 25 pounds on a regular basis;
  • Must be able to sit and work on floor; kneel and bend.
Communication:
  • Must exhibit courteous and professional demeanor towards program participants, staff and administration via verbal, written, physical, and electronic correspondence;
  • Requires good oral and written communication skills;
  • Must be able to maintain confidentiality.

Minimum Qualification Requirements

  • Bachelor’s degree in Human Services, Social Services, or related field and two (2) years of experience in working with culturally diverse population, especially with Natives from the Micronesian Nations; 
  • Working knowledge of personal computer;
  • Proficient in Chuukese, Kosraean, and English;
  • Proficient in MS software applications such as Word and Excel;
  • Must have access to a vehicle, have a valid driver’s license, a safe and responsible driving record, and current no-fault auto insurance, at least $100K in bodily collision per person and $25K in property damage liability.
